How to add a label on Form?

Hi Community,
I need to add a label or heading in Incident form which is similar to label type variable in catalog item.
I just want to put some Check box fields under it, Can any one suggest how to do that? i dont want to use annotations.
Please refer the screen shot: I need in terms of business ? as a label.

hi
you can add annotation in Incident form
go to form design -> field type annotation
change your annotation type as per your requirement

Hello Nitin,
I believe we do not have any such label type but what we could do here is we could write some UI macro here and add a radio button for yes/No.
Apart from this you can created true/false field.
