How to add a "terms and conditions" checkbox to a record producer

Hi.
I have a record producer that captures details from a potential new user.
I'd like to add a "please accept the 'terms and conditions' check box to the form" where the phrase "terms and conditions" links off to another page/KB Article.

I've spent a couple of hours googling this and have not been able to find anything.

There is no direct option to create Terms & Condtions but Servicenow Guru have a blog on this. Please go through this and let me know if you need any help

I really appreciate the swift responses, however neither of these helped.
When I create a macro with label/macro only the label shows and I still cant put HTML/a link inside of it.

I dont want to attach a PDF.

I want a mandatory checkbox at the bottom of the form with a label that says "I accept the terms and conditions" and the words "terms and conditions" are a link to another page.
