How to add Checkboxes to a email notification

Good day everyone,

i have created a SC request form and on that form we have checkboxes

I would like to have those checkboxes to appear on the email notification going out to the users on those fields

How would i be able to do this?

@Peter Williams  you can write HTML code in notifications to add check boxes.

creating textboxes will require HTML code and the code will be lengthy.

I will suggest just to add variable label and include true/false if checkbox is selected or not
