How to add Fields from the Policy Exception record form in the pop-up

Hi All,

Can anyone help me with below requirement.

Need to display below Fields from the Policy Exception record form appeared in the pop-up when we clicked on
"Request Exception" button and as of now they were added some other fields on the same popup I want to add these fields as well in the same ui page.

System Category
System Business Purpose
Number of Systems Affected
SOX
GxP
Risk Mitigation Plan

Could you not simply add the required fields to the UI page HTML in the same manner as you did for the rest of the content you've added?
