How to add values from fields to a dashboard

I have a need to add values from three (3) fields onto an existing dashboard that shows records from the current open Review Period. Data is being pulled from one table (table: review_period_table). The 3 fields are:

1. Review Name (static value, drop-down, default to recent review period)

2. Review Start Date (auto-fills when review name is selected, read-only)

3. Review End Date (auto-fills when review name is selected, read-only)

How can I achieve this? Do I create a separate widget with the 3 fields? Picture below for reference:

Hi @Phong Lo ,

In general I would say that you need a dashboard with two different widgets - either reports or performance analytics widgets based on your need. Furthermore, you need either automated breakdown or interactive filter (based on if you work with PA or reports) to filter your records accordingly.
