How to Auto Fill the fields in Request form after selecting Template in the request form

Hello Friends

My Scenario is when user selects a Predefined Template from the Template field all the predefined fields should fill automatically. In ServiceNow we have two options to auto fill, but we have a template field in the request form when user selects what are the fields defined in the template should fill. 

So please help
