How to create a new Tab in the Service Portal Request page ?

Hi all, I am looking for an efficient way to make a new tab configurate with a custom text field, mandatory and a button beside to create a relaunch of the tickets through the Portal Service interface, My requests.
I cloned the Incident Standard Tab Widget but I cannot desactivate the standard, I can't find a way to do it.
