How to create a ui action on service portal

I have a requirement to create a new button beside Service Catalog(as shown below) which will redirect to another page.

Please let me know how I can achieve this.

You can add the page in menu item as mentioned below.

So do I have to mention my URL in page which I create?

No need to pass URL, you can simply select the desired page from the reference field of page.

It worked but the page is getting opened in the same tab how to I make it open in a different tab?
