How to create custom portal page

Hello all, I'm new to portal page can anyone assist...

New custom portal page to fill form according to below fields and it should create that record in custom new table which I have created.

Fields (Editable):

  • Vendor Contract 
  • Company 
  • Is this a T&M Contract
  • Invoice Amount 
  • Period Strat 
  • Period End 

On submit, this page needs to link the user to the Reconciliation record created.
