How to display message on form?

I would like to   show   message on top of   the form, before submitting.

My form is like this.

I am new to ServiceNow please help me in this requirement.

Hi Kataru,

You can use Form Annotation in your form.

You can go to your form design and   go to field types and search for annotation and drag to your form field and add the text you want to keep in form and save the form .

Steps:

Right click and go to form design.

And go to field types search for annotation.
