How to enable give feedback on Service Portal

How do i add/enable "Give feedback on Service Portal"

Feedback Analytics > Tasks (we added a report on sn_ex_sp_pro_feedback_task table to our dashboard as well)

Tasks will only appear here if you have set up a Feedback definition to create one based on a rating threshold.  I also added the assessment_response.feedback_comment field to the feedback task form with a UI policy to make it read-only.
