How to export a completed Vendor Risk Assessment questionnaire in a specific format

When we export questionnaire responses to a spreadsheet, is it possible to export it in a specific format?

 

I want it in this format:

NO it is not possible to download the assessment in the specific format. You need to create it manually only.
