how to generate assessment surveys through workflow for catalog task

Hi all,

I am trying to configure assessment surveys using workflows in service request, in the workflow I have 1 approval that goes to manager, then 3 task are opened sequentially. After the completion of first task a wait for condition is placed that waits for the second catalog task to go into WIP or Pending State, then a run script activity that is used to trigger the assessment survey.

The survey is not getting triggered, I don't know why?

All the metric types and categories are present in the system for assessment yet it is not getting triggered.

On my best day I F$%&ING despise, loath, and hate launching Assessments.   Even when doing it one of the two "normal" ways.

Assessments and Surveys do seem to be very frustrating to work with.
