How to get a " Placeholder " in the select box or Reference ? Please help

Need help how to get a placeholder in the Select box or reference fields  like 

In place of None i need a placeholder like this

Hello Dhananjay

I have gone through that link earlier but still it is not working. Where as i kept in the question i need it like when we place a reference box or select box, and it will start with none right where i don't want none in that place and need a text which should be a place holder " What is you issue" and then when user select the box the application will come down.
