How to popup UI page on listview without using client callable option & capture the input of UI pg ?

I want to call UI page using without "client" callable UI action, also need to capture the input of the UI page on Comment field.

Que: 1. How to call UI page without Client callable ?

Que: 2. How to capture the input of the UI page in Comment field?
