How to setup "survey" for incident and service request.
I have performed below steps:
1. Created new survey from "view surveys" and designed to according to my requirement.
2. Then i have setup the trigger condition as "state is resolved" choosing table "incident.
I am not sure while setting up email notification what needs to be setup in the email notification like:
what should be table?
when to send?
whom to send?

Mahi,
When you set the trigger you would had mentioned the user field , condition and table .
so based on this trigger all your question will be answered:
what should be table? as stated in the trigger
when to send? when condition is met in trigger table
whom to send? user field as stated in the trigger

Not sure what's going on then. Try to modify the filter condition as below
Incident state 'changes to' Resolved instead of Incident state is Resolved.
Is the email notification active? Also did you verify the system logs?
Please hit correct based on impact of response.

evaluation_method=survey^active=true^publish_state=published

Can you change repeat interval under trigger condition , Days value to 00 and try again ?
