How to trigger a Change Approval Policy?

We have put in a bunch of Change Approval Policies in our test system, but the only problem is that none of them appear to be triggering.

Do I need to activate some sort of plugin to do this?  I was under the impression that if a Change Approval policy matches qualifications in a table, it triggers for all changes that match the qualifications.

Can someone please point me in the right direction on how they are supposed to be used?

Thanks Gagan, but wondering if you could explain something for me.

As a test, I decided to attach the Change Approval Policy to a Flow to see if it will trigger.  It did, as I had expected.

But when I removed the Change Approval Policy from the flow, it does not.

Is there some sort of setting that forces the Change Approval Policy to be used in flows?  I am missing something here and it doesn't quite make sense.
