How to upload a file from a Widget on a Portal

I am making a Widget to display within a Portal, this widget needs to allow users to upload a file to a table. Been trying a few different methods, but haven't figured out how to do this.

Hi Sachin,

Thanks for sharing this gitHub link, seems its a ready-made code. But are there any steps, where can I see the new widget.? I searched with SP Attachment widget on list of portal widgets but could not find one. Also if you could provide more details on how can I find the widget on one of my catalog form.? I think steps to be followed will help.
