how to use spModel in catalog client script?

Hello experts,

I want to create a popup window when catalog item is submitting.

In popup using spModel i want to create custom buttom like :-cancel and proceed without attachement

and also in which on text field that is "tell me a reason" in same popup window.

please provide some information on it....
