I am getting below error while assigning survey

I am configuring survey for Incident. I updated the survey trigger condition, created the survey questioner. When I try to Assign Survey from View Survey. I am getting error "Instance could not be created". It works fine in another instance. I think it some configuration error but not sure what. Can anyone please help on this.

Thanks for your response.



This instance is not migrated. I am configuring in my Dev instance. I   created a new survey but it not created a Assessable Records for this survey. For which I am getting error. Now it is not allowing to create new Assessable Records manually.

Try to search in UI Actions and script includes.

Haven't figure it out, but it seems to be specific to the request table for us...
