I can not figure it out why I'm receiving duplicate notificaions

Hello,

I have been trying to figuring out what I did wrong to received duplicate email notification. 

I do not know where I screwed it up.  Could some one please take a look at the following screenshots.

Hi @Ankur Bawiskar 

We would like send a notification to all approvers on RITM table if any work note has been made in the RITM by the requestor,  We followed this link, but the approver(s) keep receiving duplicates emails.  Please help.  Thank you
