In change request - state is not moving from assess to authorize state even after approver approved?

checked the flow design, In change approval policy - the output: state is empty and do not treat error is false.

Is it happening for all the Change request?

Are you using change models? If yes you will have to check if the change models are correctly cofnigured

Also check the Process flow
