Incident (INC) Assessment Response Snapshot

We are using Assessments "out of box" for Incidents. We have four custom rating questions, then a fifth question for comments/feedback.
Is there a way to provide a snapshot, per Incident, that would show the responses to each question and any comments that were provided? I can get all the data for all assessments in a single report, but managers want to see an overall snapshot for each INC.
For example:
INC0012345
Caller: Sally Stone
Assigned to: Jane Doe
Q1: Rating Response=3
Q2: Rating Response=1
Q3: Rating Response=5
Q4: Rating Response=2
Q5: Overall, I am happy with the support provided by Jane.
If not, is there a way to put a RELATED LIST on an INC that could provide all the assessment results for that individual INC?

Have you tried reporting on "asmt_metric_result" and group by source?? (if your using Evaluation method is Assessment)
