Instance security issue, and does not upgrade successfully

While setting up my PDI (Utah), suddenly the admin account has apparently no access.
I constantly get this message on pretty much every page:
Security constraints prevent access to requested page
Unfortunately I can't navigate to any page which might be useful in diagnosing this issue, because it's blocked by the very security constraint.
Perhaps unrelated, but I attempted to upgrade the instance to Vancouver several times, and it silently fails. I have no way to navigate to any debugging information either.

Hi there,
Do you know perhaps if your admin user lost its admin role? Did you create a personal user with admin role (= best practice)?
If none of this and you broke or something else broke your PDI... as you probably already know: there is no support for PDIs. So only thing you can do, request a new one.

You can try the "Manage instance password" option seen when clicking on the user in the upper-right of:
https://developer.servicenow.com/dev.do#!/home
If that doesn't work, then you'll need to follow Mark's suggestion.
