Instance was changed to old UI and not able to revert

Hi, I am having problem. My working instance switched to old UI and I don't have gear icon to just switch to UI 16. Also when I log off and log in again it looks good for couple minutes. Does anyone faced this issue? What may cause this? 
p.s. I tried incognito mode, cleared cashe, tried another browser

KB0743537 is answer to this issue, I was using sysparm_device for sending data through AJAX script include and it caused the issue

it returns me to UI 16 automatically when I log out and log in again but after sometime it throws me to old UI, I started to think that it may be caused by some script but I can't find it

Hello! How did you solve this issue? I have the same situation of usage of sysparm_device in a client script in a GlideAjax.
