Is it possible to create a Multiple choice field type on the form as described in the attached screenshot ?

Hello experts,

I have gone through other questions in the community on how to create a multiple choice list on the form, but I could not get a clear answer from those posts. A work around would be to create a List type field, but I would like to know if it is possible to create a Multiple choice field as shown in the below picture(I did some Photoshop)

Thank you. 

So does that mean its not possible to achieve a multiple choice check box as shown in my screen shot, in a FORM. 

And an alternative is to use a Glide List type ?
