Item name and show workflow link is missing in RITM

hello everyone,
in dev instance we found item name is empty along with show workflow link is missing in RITM.
Note: though REQ is approved.
What could be cause in this case, what needs to be done?

Workflow didn't attach, this would result in show workflow link missing.
Was RITM created?

Provide some screenshot where are you checking?
check the view you are in now, Hope you are in Default view not in Self Service.

yes in default view.
1. item field is empty , catalog item name isn't there.
2. under related link , show workflow missing, seems workflow is not getting attached.
in RITM.
