Item name and show workflow link is missing in RITM

hello everyone,
in dev instance we found item name is empty along with show workflow link is missing in RITM.
Note: though REQ is approved.
What could be cause in this case, what needs to be done?

Are you sure you have attached a published workflow to your catalog item?

yes its published workflow, seems like all catalog item effected
