Javascript Error in Browser Console

Hello 

When I Submit a Catalog Item Request I see Javascript Error in Browser Console.

I checked Browser Console and I could see it is because of a variable not defined in Catalog Client Script for onSubmit. I refreshed the Page and there were no Errors. This already exists in Dev Instance, and this cause no issues further just a alert while submitting the request.
