JIRA Integration with ServiceNow

Hi Team,

We are starting to do the JIRA integration, the requirement is we need to create the UI action[create jira] in existed incident, once clicked it should pop up box with 2 fields project and issue--these fields should pull details from JIRA once end user provides the details the bug should be create in servicenow .Has anyone done the same implimentation any guidence to create UI action please.
