Knowledge Popup window does not display when selecting "No"

In the ITIL view when I click on All -> Knowledge -> Home page -> Select or Search for any Knowledge Article. 

 

When someone clicks on "Helpful" -> "No" , I get an empty "Thank you " message and the feedback task is not created.

The same functionality works as expected when I click "no" on Helpful and a feedback task is created.

 

I looked and found this article but honestly not sure about the resolution.  The versions related list for the "Knowledge ArticleHelpful" widget is empty. So not sure how to revert back to OOB.

 

Any other way to resolve this issue and make the attached popup display when someone presses on "No" button .

Were you able to figure out the issue? we are also facing the same issue.
