Log PTO details with approval

Hi, what's the best solution to capture PTO details in servicenow. I need just one front end form and it goes for approval. Which ootb module or store plugin can be used ?

Hi @sarankumar3272 

You can use delegate module even though that is not 100% good but you can hide the OOTB field and create workflow  / flow for same.

Dates are already available on delegate forrm.
