Major Case / Major Case Information tab

I'm attempting to create a Major Case, but I'm not seeing the Major Case Information tab on the form. After changing the Case view to Major Case, I click on Create Major Case. As the form loads, I see the Major Case Information tab appear briefly, but then disappears once the form completes loading.

Try to give yourself, a major case role and is case is closed?
