Matrix Choice field

Hello,
I wanted to check if it’s possible to create a matrix using a choice field.
The customer is requesting a matrix-type choice field, but I haven’t done this before. Is there a way to achieve it?

1) Using Data lookup rule , we populate priority based on impact and urgency 

e.g 

2) for assessment/ score -> you can create Decision Matrix
