MIM Workbench communication task not getting triggered

HI Team
I have created communication plan definition For MIM Workbench and have put condition as shown below

But it is not getting attached after i promote incident to major incident and accept that as a major incident

Please help me how can i fix this issue

Is the communication plan set to active and of type 'Major Incident'?
Also, please note that the incident is marked as a Major Incident, so you're in the right place to send the communication.
However, it will not go out automatically—you'll need to trigger it manually
