Missing Save button on widget option schema modal

The save button has disappeared on our widget options modal.  Does anyone know how to get this button to appear?

yea i am, i'm wondering if it could be a UI Action issue but not sure which Save UI Action is linked to those modals.  Any ideas?  Thanks!
