my closed tasks are re-open by system need to stop this issue

Hi ,

  My closed service catalog tasks it automatically reopened by system and I have attached my workflow screen shot here.

where to check and how to stop this task creation,Please check and help me to resolve this issue....

if you are able to reproduce the issue at any time, consider debugging (click on the module Debug all).

Then go to a requested item and reproduce the issue, open in the meantime the catalog tasks in new tabs. Normally you should get all what is running against them on the bottom (Business Rules, ACLs, SQL, etc.)

Check at what time the state is modified and what happened at the same time (Business Rule, SQL, etc.).

Hopefully that will help you to find the cause.
