My flow runs fine when I test it, but does not run when activated

I'm working through the decision tree-based flow described here. Based on a person's points, it will set their band value based on results from a decision table. It's simple and it works fine when I test it against a record, but when I activate it and modify a record, nothing happens. There is nothing noteworthy in the log (I added an error handle to make sure of this). The trigger is created or updated. I'm at a loss what to try next.

Which table is triggering this flow ? happened to me today I forgot to attach it to catalog item 😄

It's a custom table. I think the problem was I had the trigger set to run Once. Anyway it appears to be working now.

I am having a similar issue. However, I have published, activated, and connected my workflow to an active Catalog Item. Is there a way to troubleshoot this? Perhaps by looking through some logs? Any information would be appreciated.
