Need to add link in Ui Pages

Hello,

I have created one UI pages(screenshot attached) and getting proper response(screenshot attached) but as per requirement I have to provide hyperlink for the value SIAAPP00****(which I am getting from asmt_assessment_ instance table(value) and the value is already available in the system(xyz table). can anyone help me here to add hyperlink and link will redirect it to the related record?
