Need to show checkbox to switch from Additional comments to Comments

Hi, We have a requirement to add a checkbox to switch from Additional Comments and Comments.

 

Currently we have this on the form it is missing the checkbox below the Additional comments

In my PDI, I can see a check box. May be log a case with Now Support.
