Not able to delete attachment in Service portal

Hello Experts,

I have moved the update set to QA environment after that am not able to delete attachment from the Service portal.

Here am able to edit the file name but deleting is not working.

In the Dev environment its working fine.

When clicking in the delete button in the console am getting below error.What might be the isssu.

Its really urgent for me

I got the issue in the widget dependency 'dialog-polyfill' was not added.After adding it,it worked perfect
