Notification being triggered twice on same record

Hi All,

We have created notification on requested Item (sc_req_item) table. 

and it is being triggered twice at same time. What could be the reason behind this?

@The Matrix 

check the sys_mod_count on that RITM if that Record got updated 2 times

If yes then email will go 2 times

See if any BR is having current.update() which is causing double updates to that record
