Notifications not getting triggered for few tasks

We have configured an email notification that will be triggered on Insert of new record in custom table, and will be sent to assigned to of the task. But currently this notification is only getting triggered for specific tasks when inserted. 

 

I checked the user preference, and notification is enabled as well for the users.

Can you give more details on the custom table that you are mentioning here and also the condition that is applied.

On which table this notification applied.

Please share the condition of the notification you have created.

Have you checked if the notification is triggered but is in sent-ignored state on the email logs table.

Where are you checking the emails triggered (on the email log table or on the Activity filter on the table) ?
