Once the approver Approves the RITM, approved info is not visible in RITM worknotes

Approval Events (Task) Business rule which update the RITM worknotes with "Approver approved the Task" comment is not populating for some RITMs.

This issue is impacting only few RITMs.

Do the erroneous RITMs have any shared characteristics? I'd start with that, and then check out the Debug Business Rules (Details) module. Re-approve the erroneous RITM and see why that BR is failing.
