Only Requested by's Manager can access the Catalog Item form

I have created the attached Catalog item form, I want to make this visible only for the user who is in the Manager variable (who is the Manager of the user which is present in the Name variable), could anyone tell me how we can do that?

Actually I don't want to show the form to Sourov always, it's an example. Now, I will tell you the actually requirement -

I want to send the notification to some recipients in which the user's manager also included, in that I am sending the link of the catalog item with user's info thru url parameter. So, when the manager receives the link and after clicking on that link that manager will be redirected to the portal and that Catalog item will be opened with the users info, so above catalog item is the example of this. However that same notification will be sent to some other recipients too, but those recipients are not the manger of that user so we don't need to show that form to those recipients other than the Manager.

So the user criteria will take care of this.

Even if they directly visit that catalog item the user criteria will tell you are not authorized to submit via OOB logic

What's the issue then? Your requirement is achieved using the user criteria script
