Open service catalog item without headers, navigator, and menu items

I created a new catalog item called "Create Incident". It opens like shown in below screenshot:

But I would like user to just see below (with attachment button):

Any idea how to achieve this?

Hi @MYTMPMAZA ,

If you don't want navigation, header menus ,etc. Then open the item in new tab and try it. 

or 

open the catalog item in portal so that you can see only form.

Even if I open from portal, it still shows header (navigation) as highlighted below:

Any way to remove that?

When End User will open the "incident catalog form" from Service portal then they will see only the below form not the headers, navigator, and menu items :

For the Attachment button - Open your Catalog item and  select the mentioned field:
