Open Service Portal Page from Module Without Redirecting to a New Tab

Can I open a Service Portal page directly from a module in the main ServiceNow instance, without redirecting to a new browser tab or window?

In other words, when I click my custom module, I want the portal page to load within the same window (like a normal list or form), not open as a separate page.

How can this be achieved?

You can do it as same as what I have done as shown in screenshot:

You can change your application name under which you have to see under modules for your portal menu.
