Page is refreshing and field values are getting cleared when attaching a attachment in ServicePortal

Hi
In the below form when we try to attach an attachment and submit the form on the service portal page is getting refreshed and values that are entered in the fields are being cleared. So the user needs to enter values two times.
Any solution to fix this without page getting refreshed/loaded.

Hi,
I faced a similar issue where attaching a file on a Service Portal request caused the page to refresh and clear all the entered values, forcing users to fill the field again.
