Pass INC caller to request(RITM) when user select 'create Request'

Hi All
I have a requirement as follows:
When the user selects 'Create Request' on the incident and selects a particular catalog item on the catalog form, I want the incident caller's name in place of the 'requested for' field of the catalog item.
I attached some screenshots for better understanding.

I Already check that, but the challenge is we don't know which catalog item user will select, and how to check which catalog item is selected and how to access that catalog variables to pass caller name?
