Playbook not advancing to next step after clicking mark complete

I created a playbook for our first level support for the incident form and the activity on the screenshot is a "User Form" activity.
After clickingon mark complete nothing happens, the activity is not marked as complete and the playbook is not proceeding to the next step. Does anyone know why and how to fix it?
